Understanding Primary and Secondary Headaches – And How Osteopathy Can Help

Primary Headaches
Primary headaches occur when the headache itself is the main condition, rather than a symptom of something else. They are not caused by an underlying disease.
The most common types include:
- Tension-type headaches: Often described as a band-like pressure around the head, commonly linked with stress, muscle tightness, or poor posture.
- Migraines: Typically moderate to severe, throbbing in nature, and may be accompanied by nausea, light or sound sensitivity, and visual disturbances (aura).
- Cluster headaches: Severe, one-sided headaches that occur in cyclical patterns or “clusters,” often around the eye.
Did you know? The brain itself cannot feel pain. Headache pain arises from irritation or inflammation in the pain-sensitive structures around the head and neck — including muscles, blood vessels, and nerves.
Secondary Headaches
Secondary headaches are those caused by another underlying condition. They are less common than primary headaches but can sometimes be more serious.
Examples include:
- Sinus headaches: From sinus infection or congestion.
- Headaches from head injury or trauma.
- High blood pressure headaches.
- Vascular conditions (e.g. aneurysm).
- Cervicogenic headaches: Caused by dysfunction in the cervical spine (neck joints, muscles, or nerves). Pain often begins in the neck and radiates to the front of the head, usually on one side. They are typically worsened by neck movement or prolonged posture, such as desk work.
Red flag warning: Secondary headaches that come on suddenly, severely, and without obvious cause may indicate a medical emergency. Always seek urgent care if this occurs.
The Role of the TMJ (Jaw Joint) in Headaches
The temporomandibular joint (TMJ) — or jaw joint — can also contribute to headaches. TMJ-related headaches often occur alongside jaw pain, clicking, or teeth grinding (bruxism). Because the jaw muscles are closely connected to the head and neck, tension in this area can trigger or worsen headaches.
Symptoms of TMJ-related headaches may include:
- Pain in the temples or around the jaw.
- Restricted or painful jaw movement.
- Headaches linked to clenching or grinding.
Osteopathic treatment can help by releasing jaw muscle tension, improving TMJ mobility, and addressing posture or stress factors that contribute to the problem.
